I, as a Participant in the Walk/Run sponsored by the Logan County Prevention Coalition do hereby release the aforementioned Organization from any liability under the laws of the State of West Virginia in the County of Logan, including its directors, officers, employees, and agents from any liability relating to the event.
- Waiver and Release: I release and forever discharge and hold harmless The Logan County Prevention Coalition and its successors and assigns from any and all liability, claims and demands of whatever kind or nature, either in law or in equity which arise or may hereafter arise from the aforementioned event that were provided by The Logan County Prevention Coalition. I understand and acknowledge that this Release discharges The Logan County Prevention Coalition Virginia from liability or claim that I may have against The Recovery Group of Southern West Virginia with respect to bodily injury, personal injury, illness, death, or property damage that may result from the services provided by this organization while participating in the event.
- Insurance: Further I understand that the Logan County Prevention Coalition does not assume any responsibility for or obligation to provide me with financial or other assistance, including but not limited to medical, health, or disability benefits or insurance of any nature in the event of an injury, illness, death or damage to my personal property. I expressly waive any such claim for compensation or liability on the part of The Logan County Prevention Coalition beyond what may be offered freely in the event of any such injury or medical expenses incurred.
- Medical Treatment: I hereby release and forever discharge The Logan County Prevention Coalition from any claim whatsoever which arises or may hereafter arise on account of any first-aid treatment or other medical services rendered in connection with an emergency during the event as a participant.
The undersigned agrees, on behalf of himself/herself and on behalf of any and all of his/her successors and assigns, to hold harmless The Logan County Prevention Coalition from and against all claims, demands and causes of action for personal injury, death or personal property damage, including and without limitation, all costs, expenses, and legal fees incurred in defending the same, made by or through the undersigned, or on his /her behalf, relating to or arising out of any negligent, grossly negligent and/or intentional acts on the part of the undersigned in connection with the undersigned’s entry into and participation in all activities participant of The Logan County Prevention Coalition.
The undersigned warrants that he/she has read Release and Waiver of Liability fully understand it to be a release of all claims known or unknown, present or future, that he/she has or may have against The Logan County Prevention Coalition.
I furthermore certify that I am 18 years of age or older.
